About Our Town Tomah

Our Town Tomah is a CBRF that cares for residents that have Dementia and/or Alzheimer’s disease. We recognize that each person is an individual with physical, emotional, social, and spiritual needs. Our goal is to provide care and services above the level of room and board, but less than skilled nursing care for our residents.
An environment is provided which is as homelike as possible and is compatible with the Residents’ needs and services. Residents are involved with choice of their care and preferences.

Our Founders.
Our Town Tomah was built by two families with deep Wisconsin roots and a shared belief that seniors deserve more than a place to live, they deserve a real home and a real community.

The Yahnkes and the Monroes brought together decades of experience in healthcare, operations, and hands-on caregiving to create something Tomah had never seen before. What started as a vision became a facility. What became a facility became a neighborhood.

Jason & Katie Yahnke | Ray & Jill Monroe
Jason and Katie Yahnke have been married for ten years and live in Holmen, Wisconsin with their five Daughters. Both Jason and Katie grew up in Holmen and graduated from Holmen High School. Jason and Katie’s main priority are their wonderful children. Jason and Katie grew up with their parents as entrepreneurs, so they enjoy showing their children the hard work and dedication it takes to own and operate a successful company. As a couple, they enjoy going to their daughters’ activities, attending Packer football games, boating on Castle Rock Lake, and spending time with family and friends. Jason started working in his family construction company at a very young age. Today, Jason is part owner and operator at Olympic Builders General Contractors which is located in Holmen, Wi. After high school, Katie went to achieve her Bachelor of Science Degree in Marketing and Business Administration from Winona State University. She then went to Graduate School for her MBA from Viterbo University. Together, Jason and Katie own and operate a real estate company called TEK Rentals, a vacation rental company called Castle Rock Vacation Rentals, and are partners in The Dirty Turtle Restaurant and Bar on Castle Rock Lake. Their newest venture, Our Town – Tomah Assisted Living Facility is very exciting for them. Jason has worked endless hours on the construction of the new facility to ensure it is built with the best quality. Jason and Katie are very compassionate people and know they have what it takes to help own and operate this assisted living facility.
Ray and Jill Monroe are co-owners of Our Town Assisted Living Memory Care facility. Jill is the Administrator of Our Town Tomah and a Licensed Practical Nurse. Ray has been in Operations Management for 25 years. Ray and Jill are hard working and compassionate people who have always dreamed of having their own Memory Care facility and servicing the Elderly population. Jill is an LPN and has received her Administrator license. She has managed Assisted Living Facilities for the past 20 years and has a passion for the people she serves. Ray has a bachelor’s degree in Business and Marketing. He excels at improving culture and driving efficiencies. Ray and Jill have 4 children from the ages of 30-16. Family time is very important to them, and they love that their children also enjoy being part of the business. Ray and Jill both grew up and graduated from Tomah. They love the small-town community and are so excited to serve the residents of Tomah.

Jill is an LPN and has received her Administrator license. She has managed Assisted Living Facilities for the past 20 years and has a passion for the people she serves.
Trisha Cardon - Nurse

Hello, I’m Trisha Cardon and I am the nurse manager here at Our Town Tomah Assisted Living. I’m very excited to be a part of Our Town and working with all the residents and staff. I’ve been in the nursing field for 30 years, all my experience has been right here serving the Tomah community, where we raised our two children. I enjoy hiking, cross-country skiing, kayaking, hunting, reading, and spending time with family & friends.
