CBRF - Assisted Living

The Nurse will evaluate prospective residents, and current residents, for the appropriateness of admission and continued placements taking into consideration the needs of the resident with the capabilities and limitations of the facility and staff. Current residents who are determined to be terminally ill may be retained dependent upon the total needs of the individual situation and Our Town CBRF’s ability to provide the level of care to the resident that (s)he deserves. Our Town CBRF will hold contracts with various local Hospice agencies.

Assisted Living is a residential care setting designed for individuals who value their independence but may require assistance with certain activities of daily living. These communities provide a supportive, safe, and home-like environment where residents receive personalized care while maintaining dignity, autonomy, and quality of life.

Services Commonly Provided in Assisted Living

  • Medication Administration and Management – Safe administration of prescribed medications, monitoring for side effects, and coordination with physicians and pharmacies.

  • Assistance with Personal Care – Support with showers, bathing, dressing, grooming, and personal hygiene while promoting independence whenever possible.

  • Mobility Assistance – Help with walking, transfers, and fall-prevention strategies to ensure safety and confidence.

  • Diabetic Care – Monitoring blood glucose levels, assistance with insulin administration (as appropriate), and coordination of dietary needs.

  • Housekeeping and Laundry – Regular cleaning of resident apartments and laundering of personal linens and clothing.

  • Nutritious Meals and Dietary Support – Well-balanced meals tailored to individual dietary requirements.

  • Social and Recreational Activities – Structured activities designed to encourage engagement, mental stimulation, socialization, and overall well-being.

A Focus on Compassionate Care

At the heart of Assisted Living is compassionate, person-centered care. Trained caregivers and staff members work closely with residents and their families to develop individualized care plans that reflect personal preferences, medical needs, and lifestyle choices. The goal is not only to provide assistance, but to foster meaningful relationships, emotional support, and a sense of belonging.

By balancing support with independence, Assisted Living communities strive to enhance each resident’s physical health, emotional well-being, and overall quality of life. Families gain peace of mind knowing their loved ones are in a secure environment where safety, respect, and dignity remain top priorities.
