Memory Care is a specialized form of Assisted Living designed to support individuals living with Alzheimer’s disease, dementia, and other cognitive impairments. These communities provide a secure, structured, and compassionate environment where residents receive personalized care tailored to their cognitive and physical needs. The primary goal of Memory Care is to promote safety, dignity, engagement, and the highest possible quality of life.

 

Comprehensive Services Provided in Memory Care

  • Medication Administration and Management – Safe administration of prescribed medications, monitoring for side effects, and coordination with physicians and pharmacies.

  • Assistance with Personal Care – Support with showers, bathing, dressing, grooming, and hygiene while encouraging independence whenever possible.

  • Mobility Assistance – Help with ambulation, transfers, and fall-prevention strategies to enhance safety.

  • Diabetic Care – Monitoring blood glucose levels, assistance with insulin administration (as appropriate), and coordination of dietary needs.

  • Housekeeping and Laundry – Routine cleaning of resident apartments and laundering of personal clothing and linens.

  • Nutritious Meals and Dietary Support – Well-balanced meals provided in a structured dining environment to support proper nutrition and hydration.

 

Dementia-Centered, Person-Focused Care

Memory Care communities provide dementia-centered care that recognizes the unique progression and symptoms associated with Alzheimer’s disease and other forms of dementia. Care plans are individualized to reflect each resident’s personal history, preferences, abilities, and stage of cognitive decline. Staff are specially trained to manage behaviors such as wandering, sundowning, confusion, and anxiety with patience, redirection, and reassurance.

Structured daily routines help reduce stress and provide a sense of familiarity. Consistent staffing fosters trust and meaningful relationships between residents and caregivers.

 

Enhanced Safety and Secure Environment

Safety is a cornerstone of Memory Care. Communities are designed with secured egress doors and controlled access systems to prevent unsafe wandering while still allowing residents freedom of movement within a protected environment. Layouts are intentionally structured to reduce confusion and promote ease of navigation.

Additional safety features may include alarmed exits, enclosed outdoor spaces, and increased staff supervision to ensure residents remain safe at all times.

 

Specialized Activities for Individuals with Dementia and Alzheimer’s

Memory Care programs incorporate structured activities specifically designed to support cognitive stimulation, social engagement, and emotional well-being. Activities are adapted to meet residents at their current ability level and may include:

  • Reminiscence therapy and memory-enhancing discussions

  • Music therapy and singing familiar songs

  • Sensory-based activities and tactile stimulation

  • Gentle exercise and mobility-focused movement programs

  • Art, crafts, and creative expression

The Nurse will evaluate prospective residents, and current residents, for the appropriateness of admission and continued placements taking into consideration the needs of the resident with the capabilities and limitations of the facility and staff. Current residents who are determined to be terminally ill may be retained dependent upon the total needs of the individual situation and Our Town CBRF’s ability to provide the level of care to the resident that (s)he deserves. Our Town CBRF will hold contracts with various local Hospice agencies.
